Modern method of operative laparoscopy has been successfully used in the treatment of gynecologic diseases. Today, laparoscopic surgery appled in almost all cases of gynecologic disease. 1,700 patients underwent laparoscopic surgery from August 1989 to June 1995 in Chung-Ang university hospital.
Today endoscopy has revolutionized the practice of gynecology.
It has modified the -thinking approach, diagnostic ability, and operative capability of gynecologists involved in techniques of pelviscopy. Modern methods of operative pelviscopy has been successfully applied to the treatment of gynecologic disease. Surgical techniques included usage of videolaparoscopy, bipolar forceps, endoloops, endonahts for hemostasis, Nd-YAG laser for tissue dissection, lysis of adhesions. Endo GIA 30 stapler used in 190 cases of PAVH.
The results were as follows.
1) The age -of patients were ranged from 20 to 60, mean was 39.3± 8.5
2) The previous operative history was found in 268 cases(15.8% ),operations were represent to cesarean section, adenexectomy, appendectomy, etc.
3) The operative indications were myoma uteri(770), CIN(15), ectopic pregnancy(184), ovarian tumor(427), endometriosis(137), chronic pelvic pain(32), etc(135).
4) The operative titles were PAVH(655), myomectomy(229), adnexectomy(698), diagnostic laparoscopy(69), 2nd look laparoscopy(61), adhesiolysis(120), etc.
5) The mean operative time was 89.0± 28.2min in PAVH, 87.3± 31.2min in myomectomy, 53.8± 21.7min in adenexectomy, 29.3± 19.6min in adhesiolysis. 190 cases of LAVH using the Endo GIA stapler, mean operative time was 69.8± 23.2min.
6) Mean hemoglobin change from preoperative day to postoperative 1 day was 1.98g/dl in LAVH, 1.79g/dl in adnexectomy, 1.3g/dl in adhesiolysis, 1.0g/dl in diagnostic laparoscopy.
